J.R. Martin, Founder, M.S., M.M.S., PA-C
J.R.’s inspiration comes from the example of his father, Dr. Jerry R. Martin, M.D., who lovingly toted his medical bag around the rural community of American Fork, Utah, delivering babies, performing surgeries in the hospital, and treating patients in his clinic. J.R. earned double masters degrees in Health Promotion and Medical Science at Brigham Young University and Midwestern University. He has a wide variety of professional healthcare experience, including pediatrics, family practice, orthopedic surgery and in the Emergency department. J.R. is notoriously upbeat, fun and caring. He believes humor is one of the best medicines. He’s always ready with a joke or song on his accordion. J.R. and his wife live in St. George, Utah, and are parents to four beautiful daughters.
Dr. Colby Callahan, DO
Dr. Thomas Colburn Callahan specializes in family practice and preventive medicine. Dr. Colby was born and raised in St. George, Utah. He served a two-year mission for the LDS Church in Paraguay and speaks fluent Spanish. He completed his undergraduate degree at Dixie State University and Southern Utah University and medical program at Virginia Tech. Upon finishing his residency at St. Joseph Hospital in Arizona, he returned to St. George where he now practices with his father and brother at the Callahan Clinic. He has built up clientele tremendously with his personable care. Dr. Colby is active in his church and community. Outside of work, he enjoys carpentry and spending time with his three kids and lovely wife, Hillary.
Dennis Mitchell, PA-C
Dennis has unmatched diversity and background in medicine. He earned a Bachelor of Science in chemistry and zoology from Southern Utah University before graduating from the Physician Assistant Program at the University of Utah, and is well-known and enjoyed in his community. He was one of Southern Utah’s original Physician Assistants. He has since served as President of the Utah Academy of Physician Assistants. Dennis has expertise in cardiology, family practice, surgery, and urology. Dennis is also a community religious leader and enjoys piloting ultralight aircrafts. Some of his hobbies are fishing and building things. He resides in St. George, Utah with his wife and children.
Mekael Holt, RN, MS, FNP-C.
Mekael Holt is an innovative and inspiring Family Nurse Practitioner and mentor. Mekael obtained an RN degree and a Masters of Science degree as a Family Nurse Practitioner. Mekael helps clients resolve, correct, and heal physical problems using a variety of modalities; she is a master at assisting clients in uncovering roadblocks to their success. Mekael is knowledgeable in nutrition and exercise and she enjoys seeing clients improve their health and wellness – specializing in treatment of hormonal imbalances. Mekael owned and operated an independent clinic in Southeast Idaho. She now partners with Alive & Well and assists members to live healthy and happy lives. She is also a well-known speaker and author. She now resides in Santa Clara, Utah with her husband and 4 children.
Nicole Johnson, MS, PA-C
Nicole graduated in Social Cultural Anthropology with an emphasis in Medical Anthropology, as well as Mass Communication from The University of Utah… Go Utes! She earned her Master of Science degree in Physician Assistant Studies at the University of New England. Nicole has specialized many areas across the U.S., from family practice in Alaska to occupational medicine in Maine. Nicole enjoys physical activity and thinking of creative ways for people of all abilities to do the same. Her hobbies include rock hounding among all things outdoors, photography and writing. She inspires others to recognize and fulfill their fullest potential, including her 4 children and husband. She is also the Scrabble Champion in her extended family and welcomes a challenge anytime in both her professional and recreational life!
